Population Health Management
Alerts
At the point of care, the system can identify patient-specific suggestions for screening tests, exams, and other services in support of disease management, preventive and wellness patient care. You can set up the conditions for displaying alerts; you can follow established guidelines or set your own requirements. For instance, a condition might specify that all men over the age of 50 should have an annual PSA test. Or, perhaps, that all patients with a diagnosis of coronary artery disease should have an annual lipid profile. Conditions can involve immunizations, lab results, problems, age, sex, and previous treatment (i.e., cpt codes).
When the condition is satisfied, the recommended action is displayed. When this happens, the system will document that a preventive or disease management service has been performed or that it has been overridden, and document the reasons. Because each alert is rated by importance, physicians can choose to see all alerts or only the most important ones. Who decides on the importance level? You.
You can even individualize alerts to address a patient's specific clinical situation. For example, you would want to remove an annual pap smear alert for a woman who has had a complete hysterectomy.
