Health Management Reports
A system of health management reports, both prepared and ad-hoc, is provided to identify patients who need attention.
When run for a particular patient, the HM reports can give you the information you need to manage the patient's conditions. For instance, for a diabetic patient, the report would include
- Date and value of the patient's most recent Hemoglobin AIC
- Date and result of the most recent Urine Microalbumen
- Date of the most recent pneumovax
- Date and results of the most recent lipid profile
The health management reports can also be run against the entire patient population. This information is essential to evaluate the medical practice’s performance against guidelines, and changes in its results. For instance, who among the patients with diabetes have glycated hemoglobin (HbA1c) over 8%? And which patients don’t have any value on record at all?
