Invalid Performing Physician ID - NDC (ETS)

Problem: NDC (ETS) has returned the following error message - "Invalid Performing Physician ID" for a submitted claim.

Cause: The ETS and "Sub_ID" fields of the practice file is blank or incorrect.

Solution: Locate the "Practice and Provider" file. Assure that that the column area of the file has an entry of "ETS" in the "Ins_Type" filed and "0001" (or the NDC assigned number for this doctor) in the "ID_Number" field in the column area.

This is the same number that appears in the "Sub_ID" field in the non-column area of the file.

Notes: The system gets the needed ETS information from these fields. If they are blank or incorrect an error will be generated.
