Invalid Provider Tax ID or Sub ID - NDC (ETS)

Problem: NDC (ETS) has sent the following error message - "Invalid Provider Tax ID or Sub ID".

Cause: Several possible causes:

  1. The Employer_ID field is blank in the "Practice and Provider File".
  2. The Employer ID (Tax ID) does not match what was sent to NDC (ETS) at sign up.
  3. The Sub_ID is missing in the "Practice and Provider File".

Solution: Here are the solutions:

  1. Make sure the Employer_ID field is filled in on the "Practice and Provider File". Check all records - not just the "0" record.
  2. Verify with NDC (ETS) that they have the correct Tax (Employer) ID number.
  3. Make sure that the Sub_ID field is filled in on the "Practice and Provider File". Also check to see that column area is filled in with one line that indicates ETS in the "Ins_type" field and the appropriate number in the "ID_Number" field.

Notes: The sub_id number should be 4 characters - for example "0001" for doctor #1. NDC will supply this id number.
