Diagnosis, Facility, Lab or Referring Information missing from claims

When creating claims, paper or electronic, information for diagnosis codes, facilities, labs or referring physician information is missing.

This problem occurs when the transaction file is not filled out at the time the charge was entered.

To view the transaction record put the cursor in the "D#" field on a line with a charge:

Now press <Alt+L>. You will see the "transaction" record for this line:

The claims processor will look at this record for the information it needs to fill in the diagnosis codes, the facility , lab or referring physican information. In previous versions this information was taken from the Ledger.

If the facility code, the lab code or the referring physican code is saved on the Ledger before the charge is posted that information will automatically be added to the transaction record for that charge. You can change the information on the Transaction record after it is created. If you have already created the claim the change will not appear on the claim. You will have to re-create the claim.
